Guest Waiver;

It is expressly agreed that all activities and use of all facilities shall

be undertaken by guest at guest’s sole risk. Anytime Fitness, LLC, its affiliates, and/or its franchisee(s) and their authorized designees shall not be liable for any claims, demands, injuries, damages, or actions whatsoever to guest or guest’s property arising out of or connected with the use of any of the services and facilities of the club or the grounds on which the club is located (“Claims”). By signing below, Guest does hereby expressly forever release and discharge Anytime Fitness, LLC, its affiliates, and/or its franchisee which owns the club, and the franchisee’s partners, agents and employees, from all such Claims.
